Organisational interventions designed without an adequate diagnostic frequently treat the symptom rather than the cause. Understanding what a genuine organisational health assessment looks like is the foundation of designing an intervention that will actually move the needle.

The Diagnostic Imperative
The most common error in organisational development work is the intervention designed before the diagnosis is complete. An organisation identifies a performance problem or a culture challenge, and commissions the most readily available intervention: a leadership programme, an engagement survey, a values workshop. The intervention is delivered competently. Some improvement occurs. The underlying problem persists, because the intervention addressed a visible symptom rather than the structural cause producing it.
"An engagement survey tells you that people are disengaged. It does not tell you why. An organisational health assessment tells you why. The difference determines whether the intervention that follows actually changes the engagement score or simply measures its persistence."
The Five Domains of Organisational Health
A comprehensive organisational health assessment examines five specific domains. Strategic alignment: the degree to which the organisation’s structure, processes, and daily priorities actually reflect the stated strategic direction. Leadership effectiveness: the specific leadership behaviours that are either enabling or limiting the performance outcomes required. Cultural coherence: the degree to which the actual operating culture is aligned with strategic requirements. Process and structural health: the degree to which decision-making processes and accountability structures are producing the speed and quality of decisions the strategy requires. Talent architecture: whether the organisation has the specific capabilities in the specific roles that the strategy requires.
From Assessment to Intervention Design
The value of a comprehensive organisational health assessment is not primarily in the report it produces. It is in the intervention specificity it enables. An organisation that knows precisely which domain is most constraining its performance, and why, can design an intervention that addresses the specific cause rather than the general symptom. The diagnostic also provides the baseline against which intervention effectiveness is subsequently measured. Without a diagnostic baseline, the organisation cannot know whether an intervention has moved the needle on the underlying causes of the performance challenge.
